During the early evening of 30th January 1944 this 640 Squadron aircraft took off from Leconfield at 17.30hrs to undertake an operational flight to bomb Berlin. Well before reaching the coast of occupies Europe the aircraft began to have technical problems with one of the engines, the crew opted to try and return to base on three engines and abandon the flight to the intended target. By 22.16hrs they were nearing the coast of Yorkshire but a radio message was picked up to say that the pilot was having trouble controlling the aircraft. While attempting a landing at Catfoss airfield the aircraft stalled, dived into ground and exploding on impact at 22:32hrs, around half mile south of the airfield near Catfoss Manor. Sadly all in board were killed, P/O Andrews died of his injuries at Beverley Hospital. A number of other crews later stated that they had suffered the effects of icing conditions on their aircraft and it was possible that this had caused LW513 to become uncontrollable. Historians Albert Pritchard, Ken Reast and Eric Barton were invited along to locate the crash site with local resident and East Yorkshire aviation expert Rodney Robinson around the year 2000 with permission from the landowner and located a few small fragments that confirmed the location.

Colin Tate was born on 21st February 1917 at Randwick, New South Wales, Australia and enlisted for RAAF service in Sydney.
Reginald Andrews was born on 28th October 1921 at Morgan, Queensland, Australia and enlisted for RAAF service in Brisbane.

James Cutler was granted a commission to the rank of of P/O on probation on 5th June 1943 and rose to F/O six months later.
