On 12th November 1942 this 101 Squadron aircraft was parked at Holme on Spalding Moor airfield with no aircrew on board and was having a ground test of the flap hydraulic system from being made using the hand pump when suddenly the main undercarriage retracted. It was found that prior to the test the undercarriage was not in the locked position when the hand pumping began and eventually folded.

Lancaster W4784 was delivered to 101 Squadron at Holme on Spalding Moor on 22nd October 1942. As a result of the mishap there on 12th November 1942 Cat.Ac/FA damage was the damage assessment that was repaired on site. The repair took some time. It was not returned to 101 Squadron until 20th March 1943. The aircraft failed to return from Ops to Dortmund on 5th May 1943 that saw all of the then crew killed. Cat.Em damage was recorded and the aircraft was eventually struck off charge on 31st May 1943.
