Avro 504 E4151 in the general Catterick area.
On 15th October 1918 the engnine of this 76 Squadron aeroplane choked on a flight owing to incorrect fuel / air mixture settings made by the pilot. The pilot attempted to make a forced landing but the left wing struck a tree resulting in a crash. He was admitted to Catterick Military Hospital suggesting the incident took place in this general area rather than in the Ripon or York areas where 76 Squadron had aerodome sites. The passenger may have been an observer rather than a pilot, he was an experienced engineer and had also undertaken compass adjusting and aerial navigation training courses while in the Royal Flying Corps.
Pilot - 2Lt Hubert Breeze Mills RAF. Injured.
Passenger (Observer?) - 2Lt Alfred Lewis Cooke RAF. Uninjured.
