Avro 504 D6222 near Tadcaster aerodrome.

At 17.10hrs on 17th July 1918 this No.38 Training Depot Station aeroplane was turning back toward Tadcaster aerodrome with a failing engine when it sideslipped and nose dived into the ground. The pilot had suffered an episode of momentary faintness at the controls. The location on the casualty card states Headley Bar (which is where the aerodrome was). The casualty card gives Lt Line as fainting at the controls but the service records of 2Lt Beith has him being taken of flying duties not long after this incident which may point to him actually being the pilot at the time of the crash and a later assessment sought to then remove him from flying duties. Something later occurred that saw Beith tried at general court martial at Tadcaster and him being removed from the service.
